Thyroid · Hashimoto’s
Your TSH is “normal.” So why are you exhausted, cold, and losing your hair?
You are wiped by mid-morning. You are cold when no one else is. The weight crept on, the hair is thinning in the drain, and your mood is not yours. You asked, and you were told your thyroid is fine — it is “probably just stress,” or your age. Here is what usually got skipped: Hashimoto’s, the most common cause of an underactive thyroid, is your immune system attacking the gland — and it can do that for years while your TSH still reads normal and your antibodies go unmeasured.

Finding 01 · what Hashimoto’s actually is
An immune problem wearing a thyroid costume
Hashimoto’s is an autoimmune condition: your immune system mistakes your thyroid for a threat and slowly attacks it. It is the most common reason a thyroid becomes underactive in this country. The important part is the order of events — the immune attack comes first. The gland can keep limping along and putting out enough hormone to hold your TSH in range for a long time, even while the damage is underway and you feel every bit of it.
So the exhaustion and the cold and the hair in the drain are not you being dramatic. They can be the early years of an attack nobody was looking for.
Finding 02 · why it hides
One number, checked in isolation
A standard thyroid check is often a single TSH. TSH is a useful number, but it is a late one — it is the pituitary’s signal, not a direct look at the thyroid or the immune attack on it. Antibodies, the actual marker of Hashimoto’s, frequently rise years before TSH ever climbs, and they are rarely ordered unless someone asks. So the result is “normal,” the file is closed, and the one test that would have caught it early was never run.
Finding 03 · what a fuller look reads
The fuller thyroid picture
Your thyroid is more than one number. When it is appropriate for your situation, a more complete review can look at:
- TPO and thyroglobulin antibodies — the direct markers of the autoimmune attack, and the ones most often skipped
- Free T4 and Free T3 — the actual thyroid hormones your cells use, not just the pituitary’s signal
- Reverse T3, where it adds context — a read on whether your body is putting the brakes on your own thyroid hormone
- The drivers around it — nutrient status, gut health, and stress load, which all shape how loud an autoimmune thyroid runs
Read together, next to how you actually feel, these turn a “normal TSH” into a picture you can act on. If your thyroid question is broader, start on the thyroid overview.
Patient intake · what people describe
Does this sound like you?
- Bone-deep fatigue that sleep does not fix
- Being cold when everyone else is comfortable
- Hair thinning, dry skin, and weight that will not move
- Brain fog and a mood that does not feel like you
- Good weeks and bad weeks that swing for no clear reason
None of these prove Hashimoto’s on their own. Together, they are a reason to run the fuller panel — not to be told, again, that it is just stress.

Your guide
“A single TSH closes too many charts. The antibodies were telling the story years earlier.”
You’ve sat through the rushed visits and the “everything looks normal” — we understand how much that wears a person down, and it is not how we work. Karla M. Graf is a board-certified nurse practitioner (CRNP) and Functional Medicine Academy Certified Practitioner (FMACP), with full prescriptive authority, who gives you a full, unhurried hour and reads the whole thyroid picture. She sees adults across Pennsylvania by telehealth, from our base in Beaver — and she’ll tell you honestly whether this is a fit.

Addenda · Hashimoto’s & the Clarity Visit
Can I have Hashimoto’s with a normal TSH?
Yes. The autoimmune attack, shown by antibodies, often begins years before TSH rises. A normal TSH does not rule out Hashimoto’s; it usually means the antibodies were never checked.
I am already on thyroid medication but still feel awful. Why?
Medication replaces hormone, which matters. It does not, by itself, address the immune attack or the drivers around it — nutrients, gut, stress — which is often why symptoms linger on a “normal” dose. Looking at the fuller picture is the point of the work here.
Do you replace my doctor or my endocrinologist?
No. This content is educational and is not medical advice or a diagnosis. We read the fuller picture and coordinate with your prescribing provider where medication is involved. We do not promise specific outcomes, and we will tell you if you need a different level of care.
Is this available outside Pennsylvania?
Clinical services are limited to adults physically located in Pennsylvania at the time of care.
One next step
Bring the “normal TSH” and the exhaustion it never explained.
The antibodies can climb for years before TSH ever moves. Every year they go unmeasured is a year you feel it without an answer. Let’s check what nobody checked.
